Create an Embedded Signup Form

Build a branded signup form, embed it on your website, and automatically tag everyone who subscribes.

Forms are how your website visitors become subscribers. Maildesk forms are embeddable, styleable to match your brand, and — most importantly — they can apply tags that trigger your automations.

Step 1: Create the form

  1. Go to Forms and create a new form.
  2. In the left panel, set up the basics:
    • Form Label — an internal name only you see, e.g. Summer Campaign 2024.
    • Submission Redirect — optionally toggle this on and enter a URL (e.g. example.com/thank-you) to send people to a thank-you page after subscribing.
    • Automation Tags — the tags applied to everyone who submits this form. This is the wiring between your form and your automations: add a tag like welcome-sequence here and an automation triggered by that tag enrolls every new signup automatically.

Step 2: Choose your fields

In the Form Fields panel, tick Include in Form for each field you want:

  • First Name
  • Last Name
  • Phone Number
  • Birthday
  • Address

Email is always included — it’s the one field a signup form can’t do without.

Fewer fields convert better. For most newsletters, email plus first name (for personalization) is the sweet spot.

Step 3: Style it

The Form Styling panel controls how the form looks on your site:

  • Form Background, Text Color, Button Color, Button Text color
  • Border Radius
  • Font family

The live preview on the right updates as you go — and the form’s title, description, field labels, and submit-button text are all editable inline, right in the preview. Click the text and type.

Save the form when you’re happy.

Step 4: Embed it on your website

  1. Back on the Forms page, find your form’s card and click the copy embed code button.
  2. You’ll get an iframe snippet like:

  1. Paste it into your website’s HTML wherever you want the form to appear. It works in any site builder that accepts custom HTML — WordPress, Webflow, Framer, Squarespace, or hand-written pages.

Step 5: Watch the results

Each form has its own analytics page with an interval selector (Last 24 Hours / 7 Days / 30 Days) showing:

  • Form Views over time, total views, and total submissions
  • Locations — where your subscribers are signing up from
  • Referrers — which pages and sites send you signups
  • Devices & Software — what your audience browses on

If views are high but submissions are low, try fewer fields or a clearer headline.
